Care management services
All providers must provide care management services, regardless if you’re self-managing your homecare or selected full management.
What is care management fees?
Care management is capped at 10% of your Support at Home funding. This amount is pooled across all participants, giving your provider flexibility to deliver extra support when it’s needed most, such as during a health issue or crisis, and using less when your needs are lower. This helps ensure you receive the right care at the right time.
Care managementincludes:
Regularly assessing your needs, goals and preferences
Regularly reviewing your care plans
Ensuring your care and services align with other supports
Liaising with you, your family and support services about your care
Ensuring your care and services are culturally safe
Identify and address risks to your safety, health and well-being.
Depending if you choose self-managemnet or full-care management, case management may vary in terms of hands-on involvement and delivery. Regardless of the model, all providers must ensure that safe and high-quality care and services are delivered based on your needs, goals and preferences.
