4 Easy steps to switch providers!

1 Assessing the switch

Speak with us to determine if switching providers is right for you. We’ll discuss your reasons and provide a comparison to show how managing your Support at Home funding with us differs from your current provider.

2 Initiating the switch

If you decide to proceed, we will contact My Aged Care together to reactivate your referral code and arrange a meeting with your new Care Partner to finalise your care plan.

3 Notifying your current provider

Inform your current provider of your decision to move your Support at Home funding and agree on an exit date. Once confirmed, let us know and you can select your care and services to begin with Homecare My Way on the agreed date.

4 Transitioning funds

Your current provider has 70 days post-exit date to transfer any unspent funds to Homecare My Way. During your first 70 days with us, you can utilise care and services up to your quarterly budget amount.

Once your unspent funds are transfered, you’ll be able to access this for your care and services. Your care partner will guide you through this process so you can manage your quarterly budget effectively.

What is Self Management?

If you value independence and want direct control over your care, our Self-Management model gives you just that. You’ll be able to choose, hire and coordinate your own team of service providers according to your care plan. Once registered with us, we handle all the necessary compliance checks to ensure quality care.

With Self-Management, you’re in charge of day-to-day scheduling and adjustments, while we’re always here to provide support and guidance as you transition smoothly to your new provider.
