Getting assessed for a Government-funded Support at Home can be a confusing and lengthy process.

4 steps to get assessed for Support at Home.

1 Determine eligibility

Call My Aged Care directly or contact Homecare My Way, and we can make the call together. My Aged Care will conduct an initial phone assessment to determine if you qualify for a more comprehensive in-home assessment.

2 Schedule a home visit

The Integrated Assessment Team will visit your home to assess your living arrangements, medical history, mobility and how you’re managing your daily activities. They will create a tailored support plan to determine eligibility for Support at Home.

3 Receive assessment outcome

After your assessment visit, you’ll learn whether you’re eligible for Support at Home funding. If not, you might qualify for alternative funding, such as the Commonwealth Home Support Program.

If you are eligible, you’ll be approved for funding and added to the Support at Home priority system. Please note that wait times for medium priority can range for 6-15 months.

4 Get progress updates

While waiting for your Support at Home funding, My Aged Care will send you progress updates and estimated wait times.

You’ll receive a final letter once your funding is assigned, allowing you to select a Support at Home provider and begin receiving services.
