What are care management services?
Care management services help older Australians understand, plan and manage their Support at Home services. These services offer guidance and coordination that help make sure your support is organised around your needs, preferences and goals.
Under Support at Home, care management may include reviewing your care plan and budget, arranging services, coordinating with workers or suppliers, monitoring whether your needs have changed and helping you make informed decisions about your care.
At Homecare My Way, your dedicated Customer Care Manager can help you understand your approved supports, book services, answer questions and make sure your care arrangements continue to work for you. Through our self-management model, you choose your own suppliers at rates that suit your budget rather than being locked into one pool of services. Your dedicated Customer Care Manager is always on hand to guide you, answer questions and make sure your care is working the way you want it to.
What care management services do you receive through your Support at Home/ HCP funding?
There is a core set of services under the Home Care Package (HCP) that form the foundation of good care management. These are the building blocks that help you or your loved one stay independent, safe and well at home.
Required elements of care management generally include:
- A care plan — a personalised plan developed with your Customer Care Manager that outlines your goals, needs and the services you will receive
- Regular check-ins — ongoing contact from your Customer Care Manager to make sure everything is going well and your needs are being met
- Budget oversight — keeping track of your funding so you always know what’s available and how it’s being used
- Coordination of services — making sure the right suppliers are in place and that services are running smoothly
- Compliance and quality assurance — delivering services in line with government standards and best practice care
Beyond these foundations, the specific services included in your care plan will depend on your individual pathway and what matters most to you. Some people need more hands-on support, like personal care and nursing, while others may primarily need domestic assistance or social support.
Home Care Packages have now been transitioned to the Support at Home program, which introduced a new structure for funding and service delivery. The range of services available remains broad, and Homecare My Way is here to guide you from your My Aged Care assessment through to making the most of every dollar in your funding.
