Support at Home is the latest government-funded program designed to help older Australians live independently in their own homes as their needs change. This program brings together many of the services previously included in Home Care Packages, improving flexibility, expanding services, and simplifying support for individuals and families.

Support at Home funding is tailored to your assessed needs, which may include personal care, domestic assistance, allied health, clinical care, home modifications, and more.

Support at Home Classifications Explained

There are eight classifications for Support at Home funding, each with a specific focus and allocated amount of support. Your classification is determined through assessment and ensures you receive services best aligned with your situation.

How Does Support at Home Work?

Accessing Support at Home follows a straightforward process, building on much of what people have experienced with the Home Care Package program:

  1. Register with My Aged Care: Start by contacting My Aged Care, either online or by phone.
  2. Assessment: An assessor will visit you at home to understand your needs and determine the level of support required. This step replaces the traditional Home Care Package assessment but continues to use comprehensive criteria to match services to your needs.
  3. Approval & Priority Allocation: If eligible, you will receive an approval letter outlining your support plan and be assigned a place in a Support at Home Priority System. Previously, this was referred to by many as the “home care package waitlist.” You will be notified when your funding becomes available.
  4. Choose a provider: With your referral code, select a provider—such as Homecare My Way—who will work with you to design a care plan.
  5. Start your services: Service delivery begins once your plan is agreed upon. Funding is allocated to the provider to facilitate the services identified through your assessment and care planning.

Who Is Eligible for Support at Home?

Eligibility for in-home care previously outlined under “who is eligible for a home care package Australia,” and the principles remain the same, though processes have been reviewed and refined.

To participate in Support at Home, you must:

  • Be aged 65 or older (or 50+ for Aboriginal and Torres Strait Islander people)
  • Be an Australian resident
  • Require assistance with daily activities because of health, mobility, or other challenges

How Are Supports and Services Chosen?

Once you are approved for Support at Home, a personalised care plan is developed, focusing on your unique situation. Services may include:

  • Personal care (bathing, dressing, grooming)
  • Help with housework, meal preparation, or shopping
  • Nursing care or allied health therapies
  • Mobility aids or minor home modifications
  • Transportation to appointments or community engagement

Support at Home is structured with funding classification levels matched to needs, similar to the previous “home care package levels.” You and your provider will collaborate on how your funding is best used, ensuring support remains relevant as your needs evolve.

What Is Self-Managed Support at Home?

The program also offers the option to self-manage your home care arrangements—a model based on the previous self-managed home care package model. Self-management provides:

  • Direct control over how funds are used and which service providers you engage
  • The ability to negotiate services and costs directly
  • Freedom to adapt your plan as your preferences or needs change
